GUEST PASS POLICIES:
· During our phases of limited capacity restrictions, Guest Passes will be 3 days only, limited available per month, and can only be activated by appointment.

· All Guests are required to fill out a Release of Liability Form to activate Guest Pass.

· Each guest must provide a Photo I.D. (no exceptions) and must be at least 18-years of age without a parent/legal guardian.

· Guests under the age of 18 must be accompanied by a parent/legal guardian (if age is between 12-15 years).

· Guest Passes are reserved for Local Residents (must reside within 10-miles of the Dynamic Fitness location).

· Guest Passes can only be redeemed once every 120 days.

· In order for Guest to be issued a Guest Pass, Guest must provide a Photo I.D. and sign a Release of Liability Form.

· Non Local Residents – Paid Guest Fee is:

$10 (plus tax) per day with a Member, or,

$20 (plus tax) per day without a Member

· Guest Passes can only be Activated between the hours of:

Monday – Friday [ 9am to 9pm ]

Saturday & Sunday [ 9am to 7pm ]
